FOR INDIVIDUAL CLIENT
Biuro Obsługi Klienta (BOK) [Customer Service Point] is one of the
organisational units of BIK. BIK serves the Individual Clients and prepares the
following reports for them:
- PLUS Reports
- PLUS Report with the Credit Score Information
- Statutory Information
These reports are prepared upon the request of the persons concerned pursuant to
Rules for the sharing of the personal data processed in the Biuro Informacji
Kredytowej S.A. data set. Additionally, BOK provides explanations to the
queries related to the operation of BIK and STANDARD and PLUS Reports and the
PLUS Reports with the Score Information.
Directory
PLUS report
(disclosed upon the request of the person concerned)
1. The application for the disclosure of the PLUS Report available
at BOK or from the BIK web site. The form should be filled out, printed
and signed. The Client is required to pay the full report fee, depending on the
selected option, to the bank account as indicated on the generated and attached
to the filled out form bank transfer order or payment confirmation.
Filling out
the PLUS Report application is possible during the System working hours , i.e.
7.00 - 21.00. The system is down from 21.00 to 7.00.
2. The populated
application should be submitted personally to BOK, Monday to Tuesday from 8:00 to 16:00,Wednesday from 9:00 to 17:00,Thursday to Friday from 8:00 to 16:00 or sent by registered mail to the following address:

3. The
PLUS Report will be prepared directly after the application has been submitted
to BIK or at the date specified for the receipt. The PLUS Report may be received
personally at BOK after presentation of the confirmation of the full payment of
the fee for the PLUS Report, depending on the selected option, to the BIK bank
account. The PLUS Report may be also submitted by the registered mail to the
identified address.
Payment confirmation should include the following information:
- number of account to which the payment was made,
- name and surname;
- residential address;

in the payment title: PESEL number of the Applicant with the note "PLUS".
4. The status of the application processing (e.g. whether the application has been received by BOK by mail or whether it was sent by BOK to the Client) may be verified by entering Application processing status web site and entering the unique number generated for each Client report application submitted to BOK.
Verification of the status of the PLUS Report
application processing is possible during the System working hours, i.e. from
7.00 to 21.00. The system is down from 21.00 to 7.00.

BIK s not authorised to
amend the data processed in the BIK database. The changes are made exclusively
by the bank submitting the data and only this bank may adjust or update the
data.

BIK does not provide any information over the phone concerning the data
held on the database.
PLUS Report with the Score Information
(disclosed on a chargeable basis upon the request of the interested party)
The application for the disclosure of the PLUS Report with the Score Information is available directly at BOK or from the BIK web site. The form should be filled out, printed
and signed. The Client is required to pay the full report fee, depending on the
selected option, to the bank account as indicated on the generated and attached
to the filled out form bank transfer order or payment confirmation.

Statutory Information
(is disclosed not more frequently than once in six months)
1. Fill out the Statutory Information application form which is available directly at BOK or from the BIK web site. The form should be printed and signed.
Filling out the Statutory Information application form is possible within the working hours of the System, i.e. 7.00 - 21.00. The system is down from 21.00 to 7.00.
2. The filled out form should be submitted personally to BOK, Monday to Tuesday from 8:00 to 16:00,Wednesday from 9:00 to 17:00,Thursday to Friday from 8:00 to 16:00 or sent by registered mail to the following address:

3. The Statutory Information may be received directly after the application has been submitted to BOK or a date for its receipt will be determined which pursuant to the Act will not exceed 30 days. The Statutory Information may be also sent by registered mail to the identified address.
4. The status of the application processing (e.g. whether the application has been received by BOK by mail or whether it was sent by BOK to the Client) may be verified by entering Application processing status web site and entering the unique number generated for each Client report application submitted to BOK.
Verification of the status of the Statutory Information application processing is possible during the System working hours , i.e. 7.00 - 21.00. The system is down from 21.00 to 7.00 .
